Title: OUTREACH AND SENIOR SERVICES COORDINATOR
NEIGHBORHOODS FOR ALL AGES -PILOT PROJECT
The Bon Secours of Maryland Foundation, an affiliate of Bon Secours Baltimore
Health System, works in partnership with the Operation ReachOut SouthWest (OROSW)
coalition. Our goal is to develop enduring community partnerships that are marked
by mutual collaboration, a comprehensive approach to community development and
the ability to leverage financial and programmatic resources for the community.
Our programs are focused on Housing, Neighborhood Revitalization, Philanthropy,
and Working Families initiatives to promote economic self-sufficiency.

Overview: The Outreach and Senior Services Coordinator (OSSC) works to enhance the lives of seniors residing in two communities in Southwest Baltimore. The OSSC will coordinate outreach and services as part of a pilot program aimed at allowing seniors to “age in place.” The OSSC provides residents in those neighborhoods with information about, promotes access to, and monitors the provision of local services and resources that can assist residents in maintaining or improving their quality of life as they age. The OSSC works in conjunction with the pilot project partners: Bon Secours Hospital, the Southwest Senior Center and Operation ReachOut SouthWest (OROSW). The OSSC will also work collaboratively with the Bon Secours Service Coordinator who works in Bon Secours’ five residential rental properties for seniors and people with disabilities.
Duties:
1. Help establish new pilot program that promotes ability of senior residents
to age in place by:
a. addressing residents’ basic needs,
b. optimizing their physical health and well-being,
c. maximizing independence for the frail and disabled, and
d. promoting social and civic engagement.
2. Identify, assess, select, develop and maintain referral partnership relationships
with local service resource agencies that effectively assist residents.
3. Inform residents of available resources and provide support to successfully
access services.
4. Disseminate information through multiple avenues including group presentations,
individual meetings and written communications.
5. Provide outreach to residents in the community and explain program and services
available.
6. Coordinate joint programs in the community with assistance of project partner
organizations.
7. Refer residents to appropriate providers.
8. Administrative and documentation duties:
a. Maintain timely and accurate data and documentation of all relevant interactions
with seniors.
b. Regularly report on them to internal and external stakeholders
c. Analyze and utilize outcomes data for continuous program improvement
9. Maintain individual resident files that contain appropriate information of
interactions.
10. Provide limited case-management for residents when such service is not available
elsewhere.
11. Complete other housing and resident related assignments as directed by supervisor.
Qualifications:
1. Interest in seniors and community development issues.
2. A bachelor’s degree in the field of human services (social work, gerontology,
psychology or public health preferred), plus a minimum of 3 years of related
experience, or an equivalent combination of education and experience.
3. Experience and enthusiasm in working with people of diverse backgrounds,
particularly seniors and people with disabilities.
4. The ability to identify, assess, select, develop and maintain community service
referral partnerships that assist residents in achieving their life opportunities
objectives.
5. Ability to multi-task and work with limited supervision.
6. Excellent verbal, written and interpersonal communication skills.
7. Excellent computer and technology skills.
8. The ability to establish, maintain, track, measure and report to stakeholders
the program’s objectives and efficacy.
9. Must have own transportation.
10. Ability to work atypical hours including evenings and weekends.
11. Ability to participate in and contribute to a dynamic team with ambitious
goals.
Working Conditions:
1. Varying amounts of walking and standing typical of most office jobs, including
walking up and down stairs.
2. Varying locations in and outside of the Foundation office.
3. Travel through Southwest Baltimore neighborhoods by foot and by car.
4. Contact with community residents, partner agencies, City, State and elected
officials, and Foundation staff.
5. Atypical work hours are required, entailing evening and weekend events.
